WAIVER AND RELEASE OF ALL CLAIMS

I acknowledge I am providing my services to Lakeland College as a volunteer. I understand and agree I am not entitled to and will not seek coverage or reimbursement under Lakeland College's Workers Compensation or any other Lakeland College insurance, general liability or otherwise, for any injuries, damage or loss I may sustain related to my service to Lakeland College in a volunteer capacity.

I agree to waive and relinquish all claims I may have as a result of serving Lakeland College in a volunteer capacity against Lakeland College and its directors, officers, trustees, agents, servants and employees. I do hereby fully release and discharge Lakeland College and its directors, officers, trustees, agents, servants and employees from any and all claims from injuries, including death, damage or loss which I may have or which may accrue to me on account of my volunteer services.

I further agree to indemnify and hold harmless and defend Lakeland College and its directors, officers, trustees, agents, servants and employees from any and all claims from injuries, including death, damages and losses sustained by me or arising out of, connected with, or in any way associated with the activities related to my serving Lakeland College as a volunteer.

MINOR CHILD WAIVER AND RELEASE OF ALL CLAIMS

As a parent/guardian of a minor child/ward I acknowledge s/he is providing his/her services to Lakeland College as a volunteer. I understand and agree neither I nor my child/ward is entitled to and will not seek coverage or reimbursement under Lakeland College's Workers Compensation or any other Lakeland College insurance, general liability or otherwise, for any injuries, damage or loss my child/ward or I may sustain related to my child's/ward's service to Lakeland College in a volunteer capacity.

I agree to waive and relinquish all claims I or my child/ward may have as a result of serving Lakeland College in a volunteer capacity against Lakeland College and its directors, officers, trustees, agents, servants and employees. I do hereby fully release and discharge Lakeland College and its directors, officers, trustees, agents, servants and employees from any and all claims from injuries, including death, damage or loss which I or my child/ward may have or which may accrue to me or my child/ward on account of my child's/ward's volunteer services.
